Workflow Handoff Failure Checklist
Use this when work repeatedly stalls or changes meaning as it moves from one person, system, queue, or automation to another.
Locate the boundary
- Name the exact sender and receiver.
- Name what object crosses the boundary: row, file, ticket, message, job, approval, or API payload.
- Record the expected trigger and acknowledgement.
Check for loss
- Required fields missing?
- Status semantics changed?
- Attachment or evidence link dropped?
- Owner unset after transfer?
- Duplicate jobs created because acknowledgement is unclear?
Check failure behavior
- What happens on timeout?
- Is retry idempotent?
- Does the sender know whether the receiver accepted the work?
- Is there a visible HOLD state instead of silent failure?
Repair the contract
- Define required inputs and accepted outputs.
- Make acknowledgement explicit.
- Route exceptions to a named owner.
- Keep evidence of transfer so the same failure is not rediscovered from scratch.
