Operational Evidence Checklist

Evidence is what lets the next operator verify the state without repeating the work. The strongest handoffs capture proof at the moment the change happens.

Identity

Record the exact system, customer, environment, product, job, or workflow the evidence belongs to. Ambiguous evidence becomes nearly useless when several similar jobs are active at once.

Before and after state

Capture enough of the pre-change state to know what was replaced, then capture the post-change state that proves the intended result. For high-risk changes, keep a rollback reference as well.

Machine-verifiable proof

Prefer hashes, provider IDs, receipts, logs, version numbers, query results, or exact URLs over free-form notes. A note can explain the result; it should not be the only proof of the result.

Exceptions and ownership

If anything remains unresolved, record it as an explicit exception with an owner and next action. Do not hide uncertainty inside a generic success status.
