Incident Handoff Reconciliation

Incident handoffs fail when the next operator inherits a timeline but not a verified state. Reconcile the incident around what is true now, what was changed, and what still needs action.

State the current condition first

Begin with the present state: what is working, what is degraded, and what remains unknown. Put historical detail after the current condition so the next operator does not have to reconstruct the situation before acting.

Separate actions from outcomes

List what was attempted and separately record what was actually confirmed. An action can complete without producing the intended outcome, so the handoff should never treat an attempt as proof of success.

Carry forward only real uncertainty

Record unresolved questions, ambiguous provider actions, missing evidence, and rollback risks explicitly. If an action was ambiguous, the correct next step is usually read-only reconciliation rather than a blind retry.

Name the next owner and trigger

Finish with the person or system responsible for the next action and the condition that should trigger it. This prevents the incident from turning into an open-ended monitoring task with no decision owner.
