A task is not reconciled just because someone says it is done. Reconciliation means the expected state, observed state, supporting evidence, exceptions, and next owner all agree.

Use this when “done” is not enough

Use an operations reconciliation record after a deployment, recurring report, customer handoff, incident, vendor change, or any workflow where the next person needs proof instead of a verbal status. The goal is simple: make it possible for another operator to understand what changed and decide what is safe to do next.

The six fields that matter

Capture the expected state, the observed state, the evidence that proves the observation, any mismatch or exception, the person or system that owns the next action, and the final disposition. If one of those fields is missing, the handoff is still carrying hidden risk.

What counts as evidence

Useful evidence can be a provider receipt, timestamped log, exported configuration, version hash, reconciled ledger row, screenshot, public URL, or another machine-readable artifact tied to the exact change. Avoid evidence that only restates the claim you are trying to prove.

Close the loop

A reconciliation record should end with one decision: accepted, held, rolled back, escalated, or queued for a specific next action. That decision should point back to the evidence that justified it.
